1. Provide oversight and monitoring of Comprehensive Class Member Transition Program (CCMTP) post transition policies and procedures.
2. Conduct Class member Post-transition Quality Monitoring reviews according to the current schedule.
3. Perform Class Member Home and Virtual Visits.
4. Participate in Post-transition Case Reviews with Prime Agency and UIC-CON.
5. Participate in Post-Transition Case Consultation.
6. Monitor and Evaluate non-Permanent Supportive Housing (PSH) Transitions.
7. Follow-up on Quality of life Survey.
8. Maintain accurate and complete Daily Tracking Reports and dashboards.
9. Regularly attend and engage in individual and team supervision.
10. Maintain a positive working relationship with assigned Prime Agencies to work in the best interest of every Class Member.
11. Ensure coverage needs are communicated for any needed activities during RTO.
12. Complete annual and other relevant and required trainings in a timely manner.
