• Conduct thorough initial assessments to determine hospice care needs, developing and regularly re-

evaluating individualized care plans. Continuously ensure the care plan aligns with patient and

family goals.

• Provide skilled nursing care, including medication management, symptom control, wound care, and

pain management.

• Educate patients and families on disease process, end of life care, symptom management, and

available resources.

• Work in collaboration with physicians, social workers, chaplains, home health aides and other team

members to ensure complete comprehensive care.

• Supervising hospice aides and other ancillary staff, delegating tasks, and ensuring quality and

continuity of care.

• Maintaining accurate and timely clinical records, adhering to regulatory requirements, and

participating in quality assurance activities.

• Acting as a patient advocate, ensuring their needs and preferences are respected and communicated to the healthcare team

• Conduct thorough physical assessment to evaluate patients musculoskeletal conditions, mobility, and

pain levels.

• Regularly monitor patients' vital signs, neurovascular status and wound healing.

• Perform neurological assessments

• Assist patients with mobility, cognitive function, and activities of daily living.

• Regularly check vital signs and pain management
• Assist patient with ADLS, while promoting independence and comfort.
• Provided companionship and emotional support to the patient and family.

• Patient assessments, including monitoring vital signs
• Documentation of patient care, and communicate with patients and their families
• Collaborate with physicians, registered nurses, and other healthcare professionals

• Oversee daily operations of the dementia care unit
• Supervising and mentoring nursing staff, including scheduling, performance evaluations, and providing feedback
