I would provide comfort care for then their families. I would administer medication if needed to. I would keep them as comfortable as possible.
As a caregiver I would make sure they had a safe and clean environment so they would not fall. I would cook their meals if I was working in a private home. I would assist them while taking showers. I would clean up and wash clothes and change bed linens. Assist them with transport to Dr appointments or grocery shopping.
Observed changes in patients' condition and reported any unusual findings to the RN Case Manager.
Performed personal hygiene activities such as bathing, grooming, dressing and undressing and toileting assistance.
Monitored vital signs including temperature, pulse rate and respiration rate.
Administered medications or treatments as instructed by the RN Case Manager or physician orders.
Assisted with ambulation and transfers using proper body mechanics techniques.
Provided companionship to patients while encouraging them to remain active within their limitations.
Participated in interdisciplinary team meetings regarding patient's plan of care.
Maintained a safe environment for patient care through adherence to established safety guidelines and policies.
Educated family members about disease process, end-of-life issues, symptom management strategies and community resources available for support services.
Developed relationships with patients that promoted trust, compassion and dignity throughout hospice journey.
Collaborated with multidisciplinary team members on various aspects of patient care including pain management strategies.
Coordinated with volunteers for additional support services when needed.
Facilitated bereavement counseling sessions for family members following a death.
Conducted visits in accordance with agency policy adhering to all regulatory standards.
