SBH 7 North cardiac unit and stroke management
- Continuous cardiac monitoring: Patients with chest pain should be continuously monitored with telemetry, cardiac enzyme tests, and 12-lead ECGs. RN monitor electrocardiogram (ECG) output for any arrhythmias or other abnormalities, alerting medical staff to any concerning changes.
- Vital Sign Assessment: Track blood pressure, oxygen saturation, respiratory rate, and other vital signs, using pulse oximetry and other devices.
- Patient Assessment: Perform ongoing assessments of patients' conditions, including physical exams and monitoring of any changes in their condition.
- Oxygen Therapy: Administer and monitor oxygen therapy to maintain adequate oxygen saturation levels.
- Medication Management: Administer medications as prescribed, ensuring proper dosage and monitoring for effectiveness and side effects.
- Patient Education: Educate patients and their families about their cardiac condition, medications, and lifestyle modifications.
- Communication: Effectively communicate with patients, families, and other healthcare providers about the patient's condition and care plan.
- Pain Management: Assess and manage patient pain, providing comfort and relief.
- Skin Care: Ensure proper skin care, particularly when electrodes are used for ECG monitoring, to prevent skin breakdown.
- Procedure Assistance: Assist with various cardiac procedures, such as stress tests and electrocardiograms (ECGs).
Stroke management 7 North unit nurse's Role
- Assess and admit patient to 7 North
- Reassess vital signs, blood glucose, neuro checks as per provider orders
- Monitor for Arrhythmias while on Continuous Cardiac Monitoring for 48 hours from the time of onset of symptoms as ordered
- Keep patient remains NPO until Dysphagia screening has been completed. Notify the provider of the result. if patients fail dysphagia screen, keep patient NPO
- Monitor and report abnormal lab values related to anticoagulant therapy if ordered
- Maintain patient Safety related to ADL
- Communicate consistently with the providers ensuring a seamless plan of care and follow-up care
- Provide patient and family and care and follow-up care
- Work collaboratively with interdisciplinary team
SBH 7 south respiratory unit ( Ventilator and tracheostomy patients) with responsibilities encompassing airway management, monitoring, infection control, and patient education.
Airway Management and Ventilation Support:
- Suctioning: RNs must be proficient in suctioning the tracheostomy tube or endotracheal tube to clear secretions and maintain a patent airway.
- Ventilator Settings: RNs need to understand and adjust ventilator settings based on the patient's needs, ensuring appropriate oxygenation and ventilation.
- Weaning from Ventilation: RNs play a vital role in assessing patients' readiness for weaning from mechanical ventilation, and managing the weaning process safely.
- Tracheostomy Care: RNs are responsible for performing tracheostomy care, including cleaning the stoma, changing dressings, and inspecting the tracheostomy tube for any signs of blockage or dislodgement.
- Humidification: RNs must ensure adequate humidification of the airway to prevent thick secretions and airway obstruction.
- Oxygen Saturation: RNs must closely monitor oxygen saturation levels using pulse oximetry and adjust oxygen delivery as needed.
- Assessing Respiratory Status: RNs must regularly assess the patient's respiratory status, including lung sounds, breathing patterns, and work of breathing, and promptly report any changes or concerns.
Patient Monitoring and Safety:
- Vital Signs: RNs must monitor and document vital signs, including heart rate, blood pressure, respiratory rate, and temperature.
- Cuff Pressure: RNs should monitor cuff pressure to minimize the risk of tracheal necrosis.
- Infection Control: RNs must adhere to strict infection control practices, including hand hygiene and proper use of personal protective equipment (PPE).
- Skin Integrity: RNs need to assess and manage skin integrity around the stoma and prevent skin breakdown.
- Oral Care: RNs must provide frequent oral care to prevent pneumonia and maintain oral hygiene.
- Emergency Management: RNs must be prepared to respond to emergencies, such as tube dislodgement or respiratory distress, and have the skills to manage these situations effectively.
- Accurate Documentation: RNs must accurately document all aspects of care, including ventilator settings, tracheostomy care, assessments, and interventions.
