  • Complete medication reconciliation upon admission to the Surgical Intensive Care Unit (SICU), and discharge counseling for all patients discharged during tour of duty.
  • Round daily with multidisciplinary SICU team to discuss patients on the unit, and subsequently initiate, modify, or discontinue pharmacotherapeutic agents as needed to optimize regimen.
  • Educate patients and caregivers in the pre- and post-operative settings as needed, regarding possible drug interactions, potential side effects, and optimal methods of administration.
  • Dispense and verify patient prescription orders, conducting necessary utilization reviews for all patients admitted to, and/or discharging from the SICU.
  • Subject matter expert for interdisciplinary members of the SICU care team as it pertains to pharmacotherapy decisions, dosage recommendations, and monitoring strategies for medications administered.
  • Maintain top internal and external customer satisfaction and retention by cultivating productive relationships with patients, caregivers, and other members of the healthcare team.
  • Train two pharmacy residents annually in capacity as co-preceptor for longitudinal Drug Information rotation, as well as sole preceptor for SICU rotation.
  • Train up to four APPE pharmacy students per academic year in capacity as preceptor for SICU rotation if elected.
  • Manage drug and supply inventories for the unit by conducting monthly ward inspections, and as-needed audits of the SICU Omnicell.
  • Assist with formulary management by reviewing and completing non-formulary medication consults placed for patients in the SICU.
  • Liaise with operating room staff on pharmacy's behalf to address their urgent perioperative pharmacotherapy needs.
  • Complete focused pharmacotherapy reviews annually (and as-needed) for the surgical ICU staff to maintain currency and awareness of institutional pharmacy practices, as well as educate on new/emerging medication therapies.
  • Serve as the Heart Center Pharmacist per the request of the Chief of Cardiothoracic Surgery.
  • Serve as an active member of the Residency Advisory Committee (RAC).
  • Serve as an active member of the Pharmacy Professional Standards Board (Pharmacy PSB).
  • Serve as an active member of the Pharmacy Residency Research Committee.
