Coordinate and manage comprehensive care plans for members with complex or chronic health conditions.

• Conduct telephonic outreach and assessments to evaluate medical, behavioral, and social needs.
• Collaborate with interdisciplinary care teams, including physicians, social workers, and behavioral health specialists.
• Monitor member progress and update care plans to ensure quality outcomes and reduced hospital readmissions.

• Provide , plan, coordinate,or manage,nursing care and health education for nursing home guests.
• Supervise nursing care provided by Registered Nurses (RN), Licensed Practical Nurses (LPN), Certified Nursing Assistants (CNA) and others.
• Reviews and implements all nursing procedures and systems.
• Assesses and evaluates the systems which facilitate the delivery of quality guest care and performance of staff.
• Uses a systematic approach in the nursing process to provide individualized nursing care.
• Contributes to the guest assessments (MDS/CAA's) and the development and implementation of a plan of care.
• Evaluates guests' responses to nursing interventions.
• Understands the rational for the use of medications and treatments and correctly administers as needed.

• Administration of medications , documentation and assessment.
• Communicating on a daily basis with physicians and family members regarding patients condition.
• Maintaining patients safety.
