  • Maintain and monitor incident reporting data and documentation as per the OPWDD Part 624 and Part 625 Regulations in conjunction with IMU, IRMA, the Justice Center, and the WSIR system.
  • Act as the Opengate Liaison to NYS Justice System and OPWDD.
  • Ensure compliance and the timely maintenance of the IRMA system regarding the Opengate incidents as well as the WSIR system and the RIA system.
  • Create CAP forms after an investigation has been completed and distribute them to the appropriate employee to be completed.
  • Follow up on the CAPs and completed documentation and upload them into the IRMA system.
  • Maintain an Excel spreadsheet for incidents indicating when each task and item has been completed/uploaded into IRMA and what still needs to be completed pending the receipt of the final 149 investigation. This includes the CAPs as well as the Letter of Determination from the Justice Center.
  • Schedule monthly Incident Review Committee (IRC). Update the IRC agenda and distribute it to the committee members Type the monthly IRC meeting minutes and enter them into IRMA within the three week required time frame.
  • Complete the 148 Reports and send them to the families afters an incident has occurred.
