Identify deficits in agency quality metrics and develop, implement, and lead performance improvement projects. Track, analyze, and compare data relative to safety, infection control, and quality. Participate in the development, implementation, and evaluation of departmental and system level goals. Maintain policies and procedures in compliance with State, Federal, Medicare and credentialing standards. Develops, coordinates and conducts quality orientation to new team members. Maintains agency at a continuous state of survey readiness.

Assess and evaluate patients for discharge planning needs throughout hospital stay. Collaborate with the interdisciplinary team to create a plan of care to achieve optimal health outcomes. Assist in identification and application of community resources.

Home Health: Assess and treat patients and develop a plan of care with individualized goals and interventions to promote optimal health outcomes. Communication and collaboration with the home health interdisciplinary team to work towards patient goals of care.

Hospice: Assess and manage symptoms to provide comfort and promote quality of life in patients with a life limiting illness. Education of patients and families about disease progression and hospice philosophy. Provide psychosocial support to families as they navigate grief and loss.

Provided direct nursing care to medical/surgical patients in an inpatient setting.
