  • Provide cross-system MOUD care management, reentry planning, and outcomes tracking for a complex social-medical patient population.
  • Recruit, enroll, and retain participants initiated on MAT either:
    Inpatient at ZSFG (HOUDINI), or
    During incarceration in SF County Jail (JUNO), transitioning them to OBIC Clinic post-release.
  • Deliver evidence-based interventions including motivational interviewing and contingency management to promote MAT retention and reduce overdose risk.
  • Facilitate integrated care planning across providers in ED, ACT, social work and nursing teams as well as MOUD centers for outpatient linkage and support.
  • Meet with patients for 21 visits over 6 monts to review goals, help solve barriers to assessing care as well as help navigate community programs specific to their individual needs.
  • Conduct structured follow-up at intake, 3-month, 6-month, and discharge intervals using standardized research questionnaires.
