• Provide cross-system MOUD care management, reentry planning, and outcomes tracking for a complex social-medical patient population.
• Recruit, enroll, and retain participants initiated on MAT either:
  Inpatient at ZSFG (HOUDINI), or
  During incarceration in SF County Jail (JUNO), transitioning them to OBIC Clinic post-release.
• Deliver evidence-based interventions including motivational interviewing and contingency management to promote MAT retention and reduce overdose risk.
• Facilitate integrated care planning across providers in ED, ACT, social work and nursing teams as well as MOUD centers for outpatient linkage and support.
• Meet with patients for 21 visits over 6 monts to review goals, help solve barriers to assessing care as well as help navigate community programs specific to their individual needs.
• Conduct structured follow-up at intake, 3-month, 6-month, and discharge intervals using standardized research questionnaires.
• Demonstrate competency in delivering culturally responsive services and behavioral health support to individuals with a history of substance use, incarceration, and housing instability.
• Maintain excellent documentation and contribute to project reporting, fidelity monitoring, and quality improvement.

Contracted through the California Bridge to Treatment program as a pilot program in the pediatric emergency department.

• Provided confidential, SUD-focused CRAFFT screening to all age appropriate patients
• Offered brief interventions and facilitated conversations and referrals with the onsite BERT and SW teams to seek out resources available within the patients demographic factors such as age, parental involvement, and insurance type.
• Created and led implementation of the Naloxone Distribution Project at UCSF Mission Bay, training staff and developing public access materials.
• Maintained responsive follow-up via phone and text to support post-discharge care.
• Supported individuals in education and provision of harm reduction-based engagement.
  

• Developed strong communication and organizational skills while working as a Care Coordinator for the Spruce Dermatology Clinic, managing patient care and liaising with healthcare providers
• Developed a strong sense of empathy for both patients and providers, helping to facilitate better communication and understanding between the two
• Worked closely with engineering and product teams to ensure the software was developed and maintained and new versions were implemented in a timely and efficient manner, while also providing input on the needs and requirements of patients and providers.
• Scaled with company to include work in developing the Spruce Health SaaS product. Facilitated product sales, support and patient triage specializing in mental health, women's health and substance use treatment.
