Provides holistic care to residents in a CCRC that serves over 500 residents

Provides care in all three levels of care- independent living, assisted living, skilled nursing facility

Provides evidenced-based care in specialty of geriatrics

Assesses patients to determine individual needs and develops care plans in coordination with multi-disciplinary healthcare team

Provides care that complies with organizational, federal and state guidelines

Assess, diagnosis and treats for acute and chronic illness

Order and interpret laboratory and diagnostic studies

Orders non pharmacological and pharmacological treatments as needed

Refer to specialist or therapies as needed

Collaborates with MD/specialists/nursing team/NHA/ED/Therapist/SW as needed to ensure high quality patient centered care

Acts as a leader of the clinical nursing team in the SNF, rounds daily and provides guidance

Actively participates in reducing hospitalizations, readmissions and emergency room visits

Weekly wound rounds in SNF unit

Provides End Of Life Care in all levels, EOL discussions with patients and families, POLST forms, comfort care orders, providing Palliative Care and referring to hospice when appropriate

Discharge decision making throughout various levels of care from higher to lower and lower to higher level of care

Documenting and billing

Providing education to residents formally via presentations and informally via 1:1

Providing education to staff formally via presentations and informally via 1:1

Participate in various committees: Ethics, Quality Assurance, Leadership, Signature Experience, Clinical Liaison
