Dedicated Social Worker provides exceptional professional care and implements appropriate interventions. Utilizes variety of resources to identify unique needs and assist vulnerable populations. Adept at establishing strong rapport with individuals from diverse backgrounds.
• Manage a caseload of 8 families, requiring a multitude of organizational and time management skills that ensure children’s safety while also giving parents a chance to enhance protective capacities and better themselves.
• Write case plans, court reports, action agreements, supervision plans and safety plans. Case plans drive the case by identifying the expected outcomes that must be reached before the case can be closed. Action agreements assure parents understand what services they should be involved in to help them reach those goals. Supervision plans and safety plans ensure children’s safety while in DHS custody. Court reports are written a couple weeks before a court hearing to provide updates to the court.
• Appear in court hearings and testify as needed.
• Create case notes of all activities throughout the life of a case. Review case plans, action agreements and safety plans in a timely manner according to policy.
• Face to face contacts with all parents in each case, all children, all foster parents, and collateral contacts made mostly by phone, according to policy.
