Perform initial hospice admission assessment of patient and family to determine Hospice eligibility for initiation of hospice care in home. Primary nursing responsibilities include explanation of hospice philosophy and services, patient assessment and intervention in home or facility environment including medical history, physical status and functional needs, and provide patient and family education and training of medication, diagnosis, and interventions. Assessment includes initiating plan of care with collaboration with multi discipline team including hospice leadership team, RN and LPNs, therapy, MSW, spiritual care and collaborate with Medical Director and patients chosen primary physician to communicate patient’s needs and develop strategies to effectively manage symptoms in comfort of own their home. Admissions also includes completing comprehensive assessment, obtaining consents and benefit elections and documentation on EMR, medication and diagnosis management and education with patient and designated care givers, perform wound care, foley and suprapubic catheters, nephrostomy tubes, pleurex/asept drains, and pca pumps. Prior experience as RN Case Manager managing up to 25 hospice patients and admissions preceptor. Documentation experience in HCHB, point care.

Provide primary nursing care and case management of adult patients in home or facility. Primary nursing responsibilities include patient assessment and intervention in home environment, patient and family education and training of medication, diagnosis, and interventions, plan of care and discharge planning and collaboration with multi discipline team including management, LPNs, therapy, MSW, and collaborate with physicians to communicate patient’s needs and develop strategies to improve patient’s health. Home care case management responsibilities also include completing daily and per visit comprehensive assessment and documentation on EMR, completing OASIS assessments, medication and diagnosis management and education with patient and designated care givers, perform wound care, wound vac management, foley and suprapubic catheters, nephrostomy tubes, pleurex/asept drains.

Provide Team leadership and supervision of nursing staff of 64 bed Long term care unit. Management of Certified Nursing Assistants, Licensed Practical Nurses and Registered Nurses providing continuing education, leadership and mentoring. Primary responsibilities include developing, implementing and maintaining individualized care plans for 60 + residents and coordinating care with members of multidisciplinary care team including families. Admission and discharge collaboration with interdisciplinary team. Assessment and management of complex CHD, orthopedic, sub acute and end of life residents. Complete MDS assessments and data entry. . Participation in facility CAUTI prevention study. Familiar with Casper, CMS and MDS reporting.

Provide primary nursing care to adult clients on a 24 bed acute care unit with ratio of one RN to 5 -6 patients. Primary responsibilities include assessment and intervention in patient care, patient and family education, care and discharge planning, and provide first response in critical interventions. Responsibilities also include staffing, and staff education. Primary care experience includes post op care management of GI, mastectomy and orthopedic surgical patients.

Provide primary nursing care to adult patients on a 22 bed acute care unit with a ratio of one nurse to 6 patients. Primary responsibilities include assessment and intervention in direct patient care, patient and family education, discharge planning and collaboration with hospital staff. Charge nurse responsibilities include daily chart audits, code cart checks, patient satisfaction call backs, assisting nursing staff on floor, next shift patient assignments, and conflict management. Primary care experience includes oncology, renal, orthopedic and progressive care patients.

Provide primary nursing care to adult patients in long term care facility including sub-acute rehabilitation and hospice. Responsibilities include medication administration, wound care management, medication orders, treatments, care planning and discharge collaboration.
