  • Transitional Care RN for BPCI and BPCI-A. Worked with interdisciplinary care team to develop processes that ensured patients were discharged to the appropriate next site of care. Maintained communication with qualified patients at discharge to ensure success and prevent readmission. Developed relationships with next site of care facilities and home health agencies to optimize care and establish workflows that facilitate successful transitions to home.
