Documenting, auditing and maintaining extensive written records/reports regarding service coordination and all other Early Intervention (EI) activities; Implementing and monitoring all child and family EI services within established timeframes; Performing monthly/regularly contacts with all families, providers, EIODs, school district personnel, etc. as needed; Providing families with regular social/emotional/financial/educational resources as appropriate for their needs; Leading biannual reviews and other team meetings as needed; Conducting initial and regular home visits with families as needed; Facilitating transition from the EIP to Preschool Special Education services prior to the child's third birthday
