  • Followed direct referrals for each child that has already been diagnosed or receiving some type of therapy for their developmental concerns. Review each child’s file and put together a welcome packet for the family based on the next steps and community resources available to them.
  • Contacted each parent/caregiver to introduce myself as the child’s Family Support Coordinator II and scheduled a home visit to go over the next steps, Complete the Child Find parent questionnaire, gather more information on the child to make appropriate referrals.
  • Maintained a case load of 30 or more files and complete monthly check-ins with each family monthly or as needed depending on the family’s needs. Provided community resources and referrals Complete and submitted requests for Administrative Services Organization funds to meet the family/child’s needs.
