• A review of the Student Health Record has found that there is no record of or the record is incomplete for the following immunizations that are required by Texas State Law for enrollment in public school.
• These records must be received by 2021-01-11. Please submit to the school an updated copy of the immunization record by 2021-01-11. Texas State Law allows for a one-time 30-day period from the time of school registration for a student enrolling in a new school. Following this time period, state law (Texas Legislature, Section 38.001) does not allow students to attend school who are not adequately immunized and an exclusion notice will be sent to you.
• There are two exceptions:
• 1. If the student's physician believes the immunizations are medically inadvisable, a written statement to that affect signed by the physician must be given to the school each school year.
• 2. If you have a religious objection to immunizations, you must get an exemption form from the City of Houston indicating your objections, get it notarized and provide the school the original copy.
• Please be aware that students, who have a waiver of immunization either for medical or religious reasons, will be excluded from school if there is an outbreak of a disease for which the student is not immunized.
• The following vaccines are the States minimum immunization requirements:
• DPT/DT (Series) POLIO (Series) MMR (Series) HEP B (Series) Chicken Pox (Varicella)
• DPT #1 Polio #1 MMR #1 HEP B #1 Varicella # 1
• DPT #2 Polio #2 MMR #2 HEP B #2 Varicella # 2
• DPT #3 Polio #3 HEP B #3 Had Disease-Parent /Dr. signature and date
• DPT #4 Polio #4 HEP A (Series)
