• Conducted a formulary review for the implementation of the Baxter Pediatric Syringe Pump in Jackson West.
• Evaluated and improved medication-use processes for the optimization of patient outcomes by conducting the following MUEs: anticoagulation monitoring, incidence of duplicate therapies, steroid-induced leukocytosis, and consequent antibiotic prescription patterns.
• Developed and implemented an action plan reviewing NIOSH-listed drugs in the Jackson West formulary, comparing the 2016 and 2024 NIOSH hazardous drug lists to assess and enhance safe storage practices.
• Independently reviewed patient profiles on Cerner for medication therapy, and counseled patients on potential side effects to meet goals action plan for HCAP.
• Participated in multidisciplinary MDR and ICU rounds to optimize patient care and encourage a safe transition of care.
• Journal Club presentation: Dexmedetomidine- or Clonidine-Based Sedation Compared with Propofol in Critically Ill Patients: The A2B Randomized Clinical Trial.

• Outsourced analytical laboratories for potency testing.
• Reviewed and monitored an average of 16 patients daily.
• Optimized pharmacotherapy through antibiotic stewardship, culture review, vancomycin trough monitoring, posaconazole level assessment, and targeted hepatic/renal dose adjustments.
• Identified and resolved medication-related problems, including discontinuing unnecessary stress ulcer prophylaxis, and adjusting DVT prophylaxis.
• Managed steroid therapy by ensuring proper prophylaxis, monitoring side effects, recommending appropriate taper schedules, and optimizing administration timing.
• Collaborated with the multidisciplinary internal medicine team to develop evidence-based medication regimens, recommend dosing strategies, anticipate side effects, and establish monitoring parameters.
• Applied formulary and insurance considerations to identify accessible and cost-effective regimens.
• Integrated guideline-based and clinical research evidence into daily rounds to support therapeutic decision-making and optimize patient outcomes.
• Delivered a dynamic journal club presentation, diverse weekly topic discussions on multiple disease states, and an in-depth patient case on Graves’ disease, incorporating current guidelines, clinical trial evidence, and interactive learning strategies to enhance team knowledge and clinical application.
