Create scheduled patient physical and mental assessments, chart assessments, communicate with care care displinary team for development of individualized care plans to ensure patient care is delivered effectively and satisfactory in accordance to all Medicare and Medicaid regulatory requirements.
Schedule care plan meetings with patient’s families, and interdisciplinary team members for review and updates of patient’s individual care plans.
Participation as a management team member functions, duties, and responsibilities as assigned by facility administrator and director of nursing.
