• To act as a certified assessor, meeting with individuals and/or their families in person or via remote setting to evaluate a person's everyday needs.
• To use the MnCHOICES tool to capture the person's assessment interview, client record findings and professional judgement in a cohesive document to understand the person's story, needs, strengths, preferences, values and goals.
• Review findings against federal at state program rules for Home and Community Based waivers, Community First Services and Supports, Alternative Care, Essential Community Supports and targeted case management.
• Work closely with a multidisciplinary team of human service professionals, case managers, medical providers, service providers, and legal entities.
• To record and protect confidential client data within Minnesota State systems such as MMIS, SSIS, ISDS and MnCHOICES.
• To prepare client resources such as brochures for Minnesota Home and Community Based Services, Minnesota health care program applications, local and state resources, disability certification information, releases for needed information, and more.
• To maintain an active MnCHOICES certification and improve professional skills through continued learning, active practice and consultation.

Adult Protection:

Consistently Check SSIS for new intakes

Prepare intakes for the Screening Team

Contact the reporter with any follow up questions

Run the screening meeting

Close intakes and assign to a proper social worker

Cross report to Law Enforcement

Childcare and Foster Care Licensing:

Send out licensing applications with a form letter when requested. Open the SSIS assessment workgroup the day the app is received in intake.

Process the Data Collection form to enter Background Studies. Upload clearance letters to licensor / additional information to licensor. Track and send out packets for relicensing (5 months ahead of time & due 6 weeks later).

Home and Community Based Services.

Complete a Mn Choices referral form gathering as much detail as possible about what they or the referred person is needing. (DD CM, respite, independent living, homemaking) and if we believe they may have a developmental disability (low IQ, Cerebral Palsy, Autism).

As documents are received scan into county record system – when IQ, Adaptive and physical are received notify CM and have DD team review for eligibility and planning.

Enter all Screening documents and Service Agreements for all Thomas Allen CM cases and assist DD CM with data entry in MMIS when needed.

• To run the stores website, social media, and online store.
• To fill out all necessary paperwork and keep track of all orders for the store.
• TKeep track of all stock in the store.
• To help customers when needed with wedding or event attire, retail clothing, and online orders.
• To keep a clean and organized workspace.

• To lead a team of baristas and keep the store up to standard.
• To coach, train, and work with fellow baristas.
• To do weekly tasks like ordering, filling out and keeping track of documentation, and performing any weekly updates.
• As a Barista to multi-task in a fast-paced retail environment and used learned skills and techniques
• As an accomplished Barista to prepare and deliver consistent, quality beverages with a focus on specialty coffee and espresso recipes.
• Preparing customers drinks along with creating new drinks, set up/maintain/ clean up barista area.

• Assess clients upon arrival and complete intake procedure for social services
• Help with or self facilitate resident wellness group
• Assist with discharge planning

• To examine donors' blood pressure, temperature, hematocrit, protein, injection site, and general appearance to see if safe to donate. To create, pull, and place donor files, work reception, and clean donor screening area, with required materials every day.
• To perform adequate venipuncture and address troubleshooting problems to the best of my ability, keep a clean functional work area, and provide excellent customer service.
• To prepare plasma samples properly for testing and store in designated areas
