As well as administering medication and insulin, and completing blood pressure, glucose, urine, and stool tests, and complete head to toe assessments. Participated in teaching home medical management to clients and their caregivers. Applied professional knowledge and skills, in accordance with accepted standards of clinical practice in order to facilitate problem resolution and achieve individualized goals and outcomes. Collaborated with other members of the health care team which included physicians, PT, OT, social work and care managers. The acuity of care varied from moderate to highly acute. And, to develop a therapeutic relationship, the accountability to the patient, family and members of the health-care team and the authority to develop and implement an individualized plan of care for the patient.
