• Conductes assessments to determine needs, risks, and appropriate interventions for community members experiencing crisis.
• Coordinates referrals with community agencies to connect clients to mental health or community resources to help meet community member needs.
• Provides crisis intervention support during urgent situations, de-escalating and meeting clients with empathy and compassion.
• Maintains accurate case notes, service records, and confidential documentation aligned with policy requirements.

Additional Responsibilities:

• Acts as City of Rochester Liaison with the Family Justice Center, a collaboration of domestic violence services providers including district attorneys, multiple law enforcement agencies, and other community resources including the Child Advocacy Center, Willow Domestic Violence Center and Legal Aid, providing compassionate intakes.
• Worked with the Homeless Outreach Team and on a separate Homeless Outreach Project trying to provide resources and plans to support the homeless in getting resources and achieve permanent housing.

• Provided community based crisis intervention to various crime victims and families.
• Collaborated with police officers for various types of cases; domestic violence, homicide, accidental death, suicide, parent/child conflict, homelessness, and neighbor/community conflict.
• Detailed record keeping per NYS requirements.
• Advocated for clients by connecting them with essential community resources. with various community resources.
  Continuous collateral contact to ensure safe interventions.
• Coordinated referrals with community mental health and social service providers.

• Provided in home counseling for individuals with combined physical and mental health disabilities.
• Assessed client needs and developed individualized support recommendations.
• Advised clients on independent living options, benefits, and community resources.
• Documented counseling notes, service plans, and follow-up actions accurately.
