myself.

-Performed all annual and quarterly assessments for over 160

special needs adults.

-Facilitated all Dr. appointments, including specialists, minor and

major procedures. Dental and Optometry appointments as well.

-Liaison between all healthcare providers, LPN's, and staff

regarding patient care, medications, and behaviors.

-Visited all six group homes at least once a week to do chart

reviews, check medications, and watch a medications pass being

performed by support staff.
