Guardianship Specialist II with extended caseload
Provide the overall implementation, management, planning and coordination of a comprehensive Guardianship Representative Program for Phoenix Counseling Center and services as appropriate to the wards of Phoenix Counseling Center and their families. This process includes seeing the ward as frequently as is necessary to ensure the ward’s needs are being met, extensive case monitoring, supportive counseling, decision making regarding consumer’s personal welfare and financial resources. It is also required to have contact related to the ward no less than every 30 days. This can include telephone conversations with the ward and/or the ward’s family/caregivers to monitor ward’s activities as well as on-site visits with the ward’s caregivers when appropriate In addition, face to face contacts with each ward is required every 90 days. Develop annual plan of care forward with treatment goals and progress updates. This will include an assessment and regular progress reports. Complete contact/progress notes on all wards. Oversight of both licensed and unlicensed homes for 40-50 wards on caseload. Specific caseloads are around 45 wards but Phoenix serves over 500 wards across the state of NC that may be required to cover at times due to acuity and needs. This position involved frequent involvement with Federal and Local Government as well as Legal and Judicial Systems on behalf of wards when needed.

Investigator, First Responder for Adult Protective Services where allegations of abuse, neglect and exploitation have been into question in regards to disabled adults within Burke County and occasionally investigate in other counties if there is a conflict of interest in question. Conducting information for investigation, making collateral calls, requesting records in order to make a case decision and determine if the adult is in need of protection. At times people are found to not be able to make decisions and at that time you must request court hearing in order to determine competency of some individuals. Also, supported Adult Wards of the county as a surrogate decision maker when the state of NC had declared the person incompetent. Oversee both licensed and unlicensed homes of wards that Burke County serves as guardian.
