Key Responsibilities

• Evaluate Referrals: Review and process referrals from hospitals, physicians, and families to determine eligibility for home health services, ensuring compliance with regulations.
• Coordinate Care: Act as a liaison between patients, families, and healthcare providers, facilitating communication and coordination of services to ensure adherence to care plans.
• Schedule Visits: Organize and schedule initial home visits for clinical teams, ensuring a timely start of care for patients.
• Documentation: Maintain accurate patient records in electronic health record (EHR) systems, documenting all interactions, referrals, and coordination activities.
• Patient Education: Educate patients and families about home health services, care plans, and the roles of various healthcare professionals involved in their care.
• Policy Development: Assist in developing and implementing intake policies and procedures to improve service delivery efficiency.
