Locum Tenens Assignment; Taking care of cardiac ICU patients, including post-op open heart patients (CABG, AVR, MVR, mini-AVR/MVR, thoracic aneurysm ruptures, etc), post-TAVR patients requiring CVICU, cardiogenic shock requiring support devices (Impella & iABP), unstable angina requiring iABP, heart failure exacerbation, STEMI/NSTEMI, unstable arrhythmias and cardiac arrest patients, as well as medical ICU overflow patients; recognizing and treating post-operative complications such as bleeding, reversing coagulopathy, pericardial effusion, atrial fibrillation; line placement - arterial, central and dialysis catheters; management of pressors & inotropic medications; ventilator management; working with the multidisciplinary team to provide excellent patient care; providing night and day shift coverage; no in-house intensivist at night, but available by phone.
Managing patients in NICU with ischemic and/or hemorrhagic CVAs, TBIs, spinal cord injuries, brain tumors, seizure, SAH, post-neuro intervention procedures, etc; multidisciplinary rounding with neurosurgery, neuro-IR, general neurology and stroke teams; supporting an APP driven rapid response team; taking care of medical ICU patients; line placement; ventilator & sedation management; titration of vasoactive medication; interpreting labs and diagnostic imaging; goals of care conversations with patients and family's; providing day and night coverage with in-house intensivists.
Locum Tenens Assignment; Duties included taking care of critically ill patients in the MICU, NICU and SICU settings; discussing patient care with multiple sub-specialties including vascular surgery, neurosurgery, neuro-IR, cardiology, infectious disease and nephrology; multidisciplinary rounding with nursing staff, dietary, therapy and case management; line placement, vent & sedation management, titration of vasoactive medications; providing day and night coverage; night coverage with no in-house intensivist, but available by phone.
Management of the post-operative care for cardiovascular cases including: CABG, AVR, MVR, aortic root replacements, ascending aortic ruptures, pericardial windows, convergent procedures, etc; Vascular cases including: CEA, AAA endovascular repair/stent grafting, lower/upper extremity angioplasty/stenting, fasciotomies, mesenteric stenting, etc; Thoracic cases including lung resections & thoracoscopy/VATs; iABP removal, placement of lines, titration of vasoactive medications for optimal hemodynamic support, chest tube management; taking care of post-cardiotomy bleeding, reversal of coagulopathy; ECMO management including helping to facilitate cannulation at bedside, management of ECMO if unable to wean from pump and/or if patient cannulated for higher level of support prior to transfer to an ECMO center; sent for ECMO Advantage certification to help educate colleagues about ECMO management; step-down management of patient's and facilitating discharges; effective and timely communication with the surgeons.
Range of experience in the inpatient setting taking care of general cardiology patients, patient's requiring interventional cardiology- STEMI/NSTEMI, post-cardiac PCI, post- lower extremity ballon angioplasty/stents & placement of support devices (iABP & Impella); rounded with the EP and heart failure teams; completed structural heart consults and managed/discharged patient's post TAVR, LAAO device placement, TMVR, Mitra-Clip & PFO closure; recognized and managed post- procedural complications including hematomas, retroperitoneal bleeding, pericardial effusion, tachy/brady arrhythmias, etc
Inpatient duties included rounding on general cardiology patients, writing H&Ps, consults, progress notes and discharge summaries ;oversight of Dobutamine stress echocardiograms; outpatient duties included seeing new patient consultations, routine follow-up visits, hospital follow up visits, EKG interpretation; outpatient laboratory test reviews; effective communication with patients, colleagues, physicians and office staff.
