• Conduct clinical evaluation's using ASAM criteria in NJSAMS

• Provide appropriate orientation to program

• Formulate treatment goals and objectives for successful treatment outcomes

• Manage all new admissions and discharges,completion of all intake documentation

• follow-up on engagements for clients seeking substance-abuse treatment, conducts phone screens, and assessments

• Maintain a case load of clients dealing with substance use, mental illness and other concerns, conduct and facilitate weekly individual group therapy

• Provide crisis intervention when needed, collect weekly monitored urine drug screens, document treatment and discharge plans and progress In response to treatment

• Coordinate care with local and federal agencies when appropriate

• Monitor and discuss clients phase in treatment and MAT(medical assisted treatment) dosage

• Conducted intakes, assessments, orientations, transfers, discharges, individual and group counseling

• Report to and coordinate with State and Federal agencies when appropriate

• Utilize NJSAMS criteria and ASAMS dimensions

• Kept accurate legible electronic records

• Adhere to state requirements and regulations as well as program policies and procedures

• Utilized community resources for referrals and assistance

• Administer medication, wash dry and fold laundry, prepare and serve meals, Transport patient to all medical appointments, light housekeeping, lifting from wheel chair to bed for disabled adults, provide companionship, assist with daily grooming
