A compassionate and dedicated Registered Nurse with experience and skills in providing comprehensive patient care across diverse healthcare settings, with a strong focus on patient-centered communication, critical thinking, and collaboration with healthcare teams to achieve optimal patient outcomes.
Essential Job Responsibilities
• Review daily appointment needs: packets completed and available to staff
• Review the daily nursing calendar and schedule, and attend if possible. Ensure coverage
• Monitor staff, as needed (i.e., medication administration, meals {modified texture, therapeutic}, ADLs, implementation of the plan(s), and interactions between staff and clients
• Review incident reports – provide assessment and follow-up
• Review findings and recommendations from appointments. Implement changes of orders. Complete notes of services provided.
• Approve orders for prescribed medications/treatments(QuickMar)
• Monitor health status/vitals/seizure disorders
• Address acute ongoing health issues including "Change of Condition" documentation
• Attend IPPs as scheduled
• Review MARs for completeness and accuracy
• Provide on-call service. Manage resources
• Perform PRN Assessments: falls risk, skin integrity, choking
• Review the IPP schedule, and submit assessments 2 weeks prior to IPP
• Review follow-up appointments on the calendar
• File health care documents
• Review medication storage- cleanliness, organization, no out-of-date drugs
• Review narcotics count sheets
• Order medications that only have 7 days remaining
• Bi-weekly orientation training as scheduled
• Complete Monthly nursing summary. Do Monthly reconciliation
• Review Med exchange at assigned locations
• Develop a Medication Support plan. Update as needed
