Working with Tarhell Homehealth care, I visit elderly in their homes providing health with ADLs, errands, appointments, cooking, cleaning

I was employed by Mr. Coleman Ross to care for his son Andrew Ross as he was diagnosed with ALS. I brought Andrew home from the hospital after receiving his trach. I cared for Andrew on a 24 hr basis 7 days a week. I provided medication, bathe, shaved, transferred, transported, toileting, bathing, trach care, trach change, monitored breathing via trilogy, cough assist, nebulizing, suctioned, maintained all equipment, ordered medical supplies, prescriptions, cleaning on a daily basis.

I was employed by Mr Davis and his family to provide care for him as he was diagnosed with ALS. I worked 7 days a week for the Davis family. I provided medication, showering, hair cutting, feeding by feeding tube, toileting, cough assist, catheter care, transport, transferring, skin care, comfort, and companionship on a daily basis.

• As an employer with Pacific Staffing, my responsibilities are to assist my clients with ADL's, prepare their meals, light housekeeping, some errands, making them comfortable, providing companionship, and keeping them safe.

• Mr.
• Northcutt's wife was diagnosed with ALS in August of last year.
• I was employed by Mr.
• Northcutt to provide care for his wife in their home.
• The disease has progressed has restricted Mrs.
• Northcutt from doing anything on her own.
• She was hospitalized in March to have a trachea tube and g-tube inserted to help with feeding, talking, and breathing.
• I am responsible for helping Mrs.
• Northcutt get her showers, get dressed, eating, suctioning due to secretions, brushing her teeth, combing her hair, light housekeeping.
• Since the surgery, we have LPNs on staff as well to help with cough assist and trachea care and change.

• I was employed by Ms.
• Splawn to provide care for her in her home.
• Ms.
• Splawn had a stroke in January of this year which hindered her ability to stand or walk, as well, the left side of her body has damaged nerves.
• She has no ability of using her left arm or leg.
• She is losing her hearing, she is incontinent, and has dementia.
• My responsibilities were to change her, transfer her using a Hoyer lift, feed her, remind her of her meds, attend hair appointments and PT appointments with her.
• At times she would suffer with sundowning due to the dementia.
• As well, I was responsible for putting her to bed and keeping her shifted to avoid bed sores.

• As an employer at the Oaks of Mayview, my responsibilities were to care for the residents.
• My responsibilities included, but were not limited to assisting with ADLs, feeding, changing, companionship, vital signs, reporting of changes in behavior, skin conditions, urine, bowel movements, bathing, transferring.
• The Oaks of Mayview is a skilled facility, that provides total care on many occasions.

• As an employer of Franklin Manor, my duties included, but were not limited to, assisting residents with ADLs, feeding, dressing, bathing, laundry, vital signs, transferring, reporting of changes in behavior, attitude, urine, bowel movements.
• Franklin Manor is a total care facility specializing in Memory Care such as Alzheimers and Dementia.
