* Provide screening, brief intervention and referral to treatment (SBIRT) for Trauma Service patients who have screened positive for alcohol/drugs or have a history of substance misuse and abuse.

* Facilitate motivational interviewing to help identify patient goals and motivation for change in regards to substance use.

* Collaborate with Trauma APP and unit Care Coordinator to support patient discharge plans.

* Worked with multidisciplinary team to identify needs and barriers to facilitate patient discharge.

* Responded to critically ill patients entering the ED to help identify and locate a medical decision maker and provide support to families.

* Provided case management to children in HCDSS custody while also working with their caregivers to develop a safe and permanent living environment.

* Direct work with caregivers to establish service plan goals aimed at eliminating safety risks which resulted in HCDSS custody.
* Conducted Child and Family Team Meetings to develop community partnerships and collaboration between the family and various service providers.
* Prepared legal documents, court reports, and other agency paperwork in compliance with state and agency statutes and policies.

* Provided mental health consultation and support to referred students, families and schools, K-12th grade.

* Crisis intervention and assessments in the school and home setting.

* Advocated for students' mental health needs within the school setting.

* Completed intake assessments and developed treatment plans and linked with community resources.

* Facilitated groups to promote healthy social, emotional and behavioral development.
