My career goal is to continue to grow in nursing experiences and skills. My past nursing experiences have been instrumental in my knowledge and growth. I have gained skills; both clinical, as well as diplomatically, in dealing with my patients, their families and physicians. My desire is to continue to experience new opportunities while maintaining my clinical/assessment expertise.

This position is responsible for screening patient referrals from hospitals, skilled nursing facilities and other facilities for potential admission into one of the programs at St. Camillus, including short term rehab, traumatic brain injury and long term care. The Intake Screener will complete a thorough review and timely transfer of medical record information to expedite the admission decision making process and represent all St. Camillus inpatient programs while building strong relationships with the hospital care coordination teams in an effort to maintain census within our facility.

As the Case Manager for Alzheimer/dementia patients, I managed residents and their families form the admission process through everyday medical management during their stay. This included, but was not limited to nursing assessment, scheduling physician appointment, medication review/clarification, family care planning, concise documentation, making referrals to SNF and site evaluation for admissions. The case manager maintains concise and professional relationships between family members, medical professionals, home care agencies and facility staff. I acted as supervisor to a 69 apartment building on a regular on-call basis to respond to any emergency situation. My duties also included supervision of direct care staff and licensed practical nursing staff, as well as CNA's.
