• Receive calls from field staff and physicians regarding client care issues/orders. Enter orders as appropriate and update the plan of care, medication profile and schedule. Enter information regarding all telephone calls into the coordination notes section of the computer software. Notify staff, as appropriate, to ensure proper coordination of care.
• Review and approve all interim orders before they are sent to physician. Notify Clinical Manager when unable to approve the order.
• Provide support to the Intake staff and process referral requests in HCHB as needed. In branch offices, processing referrals is a shared responsibility of the PSC and CFSS roles.
• Assign Licensed Professional (LP) to conduct the evaluation visit as the task appears on the action screen
• Participate in case conferences, as needed, and process recert/discharge decisions based on clinical documentation and input from case manager, based on case orders for tracking purposes and route to Medical Records to be sent to the physician for signature. Ensure any corrections are made by the LP who wrote the order prior to entering the order. Update the client’s medication profile and schedule as applicable; via the order. Ensure all orders that address frequency have appropriate calendar modification completed.
• Follow-up on orders from the Order Tracking Report when the MRS is unable to retrieve the unsigned order.
• Review and follow-up on entitlement verification issues.
• Verify/Ensure there are orders for requested medical supplies, enter supply orders and requisition supplies as needed.
• Enter non-admit information into HCHB, with details in coordination notes if no visit was made.
• Review missed visit reports, ensure MD notification has occurred, and if not, notify MD

Job duties include conducting nursing assessments, performing skilled nursing functions including but not limited to, vital signs, blood draws, urine screens, EKGs, PPDs, breathalyzers, etc; medication administration; performing bed checks/ security rounds as scheduled providing patient education in individual and group settings; assisting the multidisciplinary team in developing / modifying patient treatment plans; conducting patient and luggage searches; providing medication education; preparing discharge paperwork; etc. Must ensure quality, timeliness and accuracy of EMR documentation and ensure compliance with CARF standards, NYS OASAS regulations, other State and Federal regulations, referral source and insurance contract

This position is responsible for screening patient referrals from hospitals, skilled nursing facilities and other facilities for potential admission into one of the programs at St. Camillus, including short term rehab, traumatic brain injury and long term care. The Intake Screener will complete a thorough review and timely transfer of medical record information to expedite the admission decision making process and represent all St. Camillus inpatient programs while building strong relationships with the hospital care coordination teams in an effort to maintain census within our facility.

As evening supervisor for 6 of 12 LTC/rehab cottages (72) residents, LPN's, CNA's.

Performed daily rounds, assessing acute issues and emergencies; communicating with providers on call, transcribing new orders in EMR, communication with family members. Responsible for timely completion of all A&I reports, interpreting lab and xray results to providers, communicating directly to DON and ADON with campus concerns. Primary nurse for orthopedic surgeon in office setting.

Duties included assisting physician during patient sessions, taking histories, ordering appropriate xrays, assisting with injections, removed casts and sutures/staples assessing wound/incisions, dressing changes, and patient education

Answer triage calls for six surgeons and two physician assistants.

As the Case Manager for Alzheimer/dementia patients, I managed residents and their families form the admission process through everyday medical management during their stay. This included, but was not limited to nursing assessment, scheduling physician appointment, medication review/clarification, family care planning, concise documentation, making referrals to SNF and site evaluation for admissions. The case manager maintains concise and professional relationships between family members, medical professionals, home care agencies and facility staff. I acted as supervisor to a 69 apartment building on a regular on-call basis to respond to any emergency situation. My duties also included supervision of direct care staff and licensed practical nursing staff, as well as CNA's.

Worked as registered nurse in a small rural community hospital after receiving Board licensing.
