Conduct weekly home visits with families, weekly lesson plan in partnership with parents, collect, screen and track all medical examinations, provide developmental and sensory screenings, provide appropriate infant/toddler socialization to families, develop an individual goal-oriented plan, case conferencing, referrals to appropriate services agencies, maintain documentation of all screenings, in-service trainings, plan and implement activities for parent support, input health, program information report data and notes in Child plus, and recruitment throughout ventura county, other duties as assigned.
