Provide homecare Physical Therapy services to homebound patients residing in Long Island, Queens and have also provided services in all areas of Manhattan. • Provided PT Homecare for patients in Queens and Long Island. Evaluated and treated wide range of conditions, including chronic and acute pain, disc prolapse, whiplash, dislocations, arthritis, Multiple Sclerosis, Parkinson's, stroke and neuropathies.
• Observed and recorded patients' responses to therapies and exercises to report to practitioners and recommend adjustments to treatment plans.
• Planned and administered range of treatments and therapeutic exercise programs involving active and passive ranges of motion and muscle reeducation.

Provides management of all aspects of rehabilitation services for the Sunrise of Fleetwood (Mount Vernon, NY) and Sunrise of Crestwood (Yonkers, NY) Assisted Living Facilities as well as for maintaining a clinical caseload.  Promotes an environment that ensures superior clinical and fiscal integrity.  Serves as an educator, liaison and mentor to interdisciplinary team members and the ALF staff.  Analyzes, interprets and achieves budget expectations on a consistent basis.  Communicates openly and timely and interacts with others including rehabilitation staff, community staff, residents and families in attending to and meeting their requests.

Provide Home Health Physical Therapy services via Riverspring Certified Home Health Agency serving Westchester County, NY.

• Home Health PT covering Queens area servicing Senior and pediatric population.
• Delivered home care physical therapy to homebound patients.

• Worked full time providing Physical Therapy services to the homes of patients residing in the Hyattsville, MD area.
• Patient's assessments are monitored in addition to patient/caregiver training, quality care and discharge planning.
• Monitored PTAs and HHA and delegated the appropriate plan of care.
