Self motivated documentation specialist. I strive to support and coach the clinicians in the field caring for our patients. I audit long level of stay patients looking at continued eligibility, care center, continuous care and hospitalized charts to ensure GIP level of care. All admits are reviewed for the HIS information. If deficient areas are found, I send these back to the nurses for correction. By doing this we have been at 100% for the last 3 years. Any area of documentation deficiency found company wide, I create a Quality tip with step by step instructions on how this area should be done.

I hold quarterly Safety meetings for management to discuss fall, infection and medication variance rates. If we are above benchmarks in any area, I create sub-committees to problem solve to correct this.
