  • All QAPI reports were analyzed and then placed into action to improve the care of our patients
  • I also Case Manage our Remote Patient Monitoring program thru Vivify
  • This program allows the collection of patient’s vital signs trends and then allows alternatives instead of going to the ER for non-urgent care
  • This is accomplished by assessing their needs thru triage and conferring with their MDs to obtain order to prevent a hospital stay
  • This program has become a full-time position effective September of 2022.
