Coordination of all aspects of care for the transplant patient population. Assess and plan with the patient, caregiver and multidisciplinary healthcare team to meet their needs across the care continuum. Assess health and psychosocial data reflective of the patient's status in all phases of transplant and appropriately interpret information relative to the patient's age-specific needs. Provide patient and caregiver education on a continuum relative to their needs based on assessments of current knowledge, cultural background, level of education and ability to participate. Participate in onboarding of new staff members including collaboration, assessment, support and provision of resources necessary to successfully train and advance skills according to current experience and knowledge base for the purpose of delivering safe and effective patient care. Active involvement in community, committee, quality improvement and research.

RN Staff - Coordinate and provide care to patients during the pre-transplant and post-transplant phases, including prep for surgery and post-anesthesia care for patients who are receiving or donating organs.

Planning, implementing, evaluating and communicating all phases of nursing care for assigned patients related to pre-transplant and post-transplant phases involving kidney, liver, lung and pancreas transplants.

Provide patients/caregivers/families education for transition of care post-hospital discharge.

Team Lead - Overseeing, collaborating with and supporting a team of nurses on the transplant unit; assigning patients to RN staff members overseeing their care according to skill set, experience, patient acuity and available resources; delegating tasks, monitoring patient flow, coordinating staff schedules, and acting as a resource within the team.

Provide care to patients during the pre-transplant and post-transplant phases, including prep for surgery and post-anesthesia care for patients who are receiving or donating organs. Prepare patients and caregivers for the transplant process, providing education on the procedure, immediate post-transplant care, expected recovery and associated risks.
