• Clinical Caseload Management: Provide direct, hands-on nursing care and Targeted Case Management (TCM) for a 20-patient caseload with complex co-occurring medical and mental health diagnoses.
• Advanced Assessment: Conduct comprehensive clinical assessments, medication reconciliations, and health screenings within the MN BHH framework to identify and mitigate acute medical risks.
• Acute Care Coordination: Facilitate seamless transitions of care between inpatient units and community settings, ensuring continuity of treatment plans and reducing hospital readmission rates.
• Crisis Intervention: Act as the primary clinical responder for behavioral health crises, utilizing de-escalation techniques and acute nursing judgment to stabilize patients.

Daily job duties are to care for multiple patients and staff members keeping up with medical records, and monitoring patient status to provide the best quality of care possible.

• Used first-hand knowledge and clinical expertise to advocate for patients and enacted prescribed treatment strategies.
• Managed care from admission to discharge, including patient assessments, care planning, health education and discharge support.
• Provided comprehensive care to multiple patients on the unit.
• Addressed disruptions in patient care, including delays in discharge, postponed procedures and discharge equipment unavailability.
• Identified care needs of individual patients and coordinated responses based on physician advice, insurance limitations and procedural costs.
