In this role, I oversee the care of individuals who are members of CCA One Care. I conduct home visits with my members to determine what care they require to assist them in remaining in their homes, and as independent as possible. I complete scripts following home visits, and enter those scripts into Guiding Care. I communicate regularly with the member's CCA care coordinator. I complete daily office tasks, including phone duty, regularly responding to emails, and assisting members with urgent and non-urgent needs. I currently have a caseload of 139 members. I prioritize the needs of my members, and help make sure they are receiving all the services they require.
I oversaw the community-based day services at a day program for adults with developmental disabilities. I created a daily curriculum for the individuals, organized community outings, and assisted in supervising about 8 to 10 job coaches. I also performed daily office tasks, completed scheduling for all job coaches, and completed paperwork for my caseload. I was also the Human Rights Coordinator for the agency. In this role, I ran quarterly meetings with the Human Rights Committee to ensure the individuals' behavior plans were being executed correctly.
