  • As a current case manager I provide initial and ongoing patient evaluations to determine the appropriate eligibility status for care services, as determined by the patient's physician.
  • Evaluations are conducted via a comprehensive assessment of the patient's medical history, physical exams, psychosocial needs, socio-economic status, family dynamics, rehabilitative and discharge planning needs.
  • This includes a complete review of the patient's medication regimen.Emphasis is placed on the current ordered medications; for efficacy, compliance or noncompliance with meds, actual/potential side effects, ineffective therapy, duplicative therapy or possible drug interactions.
  • All covered diagnoses are reviewed to use to implement appropriate care services.
  • As a case manager, I plan, implement and evaluate ordered care services based on the orders/recommendations of the primary MD. Using the nursing process, type and frequency of services to be provided are completed. Consideration is given for the fuctional limitations, permitted activities( ADL'S), safety measures and care goals of the patient. Care levels: RN/LPN's home health aides(CNA'S) and personal care attendants( PCA'S) are determined, reviewed and revised based on ongoing clinical evaluations.
  • Focus collaborates with other health care providers. Determining the need for PT therapists, social services, pastoral care, and supportive modalities for patients and their families. Community resources are initiated where appropriate and available.
  • Additionally, I manage health care personnel to ensure continuity and quality of care in the home setting.
  • I participate in quality control projects and perform performance evaluations of employees.
