- As Case Management (April 2005-2015), I provided services for individual in a sub-acute setting and outpatient settings including service planning and crisis prevention. Coordinated with clients and their families, physicians, and judicial system (for those with legal charges). Acted as crisis responder working with mobile crisis and emergency room.
