Caring and dedicated Registered Nurse seeking a position in a hospital, health care clinic, or other medical facility to apply excellent work ethic, extensive practical nursing experience, and provide quality care to patients and their family members.
· Conduct comprehensive assessments of residents' health and care needs upon admission and regularly thereafter.
· Develop individualized care plans in collaboration with the interdisciplinary team and residents' families.
· Administer medications and treatments as prescribed, ensuring accuracy and adherence to protocols.
· Monitor and record vital signs, symptoms, and changes in residents' conditions.
· Provide assistance with activities of daily living (ADLs), such as bathing, dressing,
· feeding, and mobility.
· Promote and maintain a comfortable and clean living environment for residents.
· Perform wound care, catheter care, and other nursing procedures following established
· protocols.
· Monitor and manage residents' pain, using appropriate pain management techniques.

· Document all care interventions, assessments, and resident interactions accurately and timely in electronic health records.
· Report any changes in residents' conditions to the charge nurse or physician promptly.
· Participate in team meetings and care conferences to update care plans and discuss resident progress.
· Adhere to infection control practices, including hand hygiene, use of personal protective equipment (PPE), and isolation precautions when necessary.
· Promote a safe and secure environment for residents and staff by identifying and mitigating hazards.
· Provide education to residents and families regarding their conditions, treatments, and self- care techniques.
· Mentor and support Certified Nursing Assistants (CNAs) and other staff members.
· Stay current with best practices in nursing and attend continuing education programs as required.
· Ensure compliance with all state and federal regulations governing nursing homes and long-term care facilities.
· Participate in surveys and inspections, assisting in the preparation and implementation of corrective action plans as needed.
· Perform required skills (assessment, intervention) according to guidelines, policies, and procedures.
· Participates in interdisciplinary committees that influence or determine policies affecting nursing practice.
· Follow facility procedures/protocols for admitting residents transferring to hospitals/clinics/outpatient visits and completion of appropriate documentation based on the established guidelines.
· Makes rounds on all resident rooms and ensures that the L.P.N.s, C.N.A.s, and/or others under his/her supervision receive their resident reports, and have their assignments and break times scheduled.
· Receive and give reports from and to outgoing and incoming shifts. Count or ensure the counting of controlled drugs with outgoing and incoming shifts.
· Develop a care plan that demonstrates knowledge of criteria to ascertain resident-specific needs based on health care problem(s).
· Makes needed revisions and updates to the care plans.
· Demonstrate ability to coordinate and collaborate with resident/family and other
· members of the comprehensive health care team.
· Monitors the administration and accurate recording of medications used.
· Notifies physicians of pertinent observations and reactions, incidents, and
· abnormal laboratory and x-ray results as required and documents same promptly.
· Report all resident, visitor, and work-related accidents/injuries and demonstrate appropriate follow-through by investigating, documenting, and processing incident reports.
· Accepts guidance, supervision, and criticism if needed.
· Assures that a stock level of medications, medical supplies, equipment, etc. are
· maintained at adequate levels at all times.

· Transported patients to locations outside the home, such as to physicians’ offices or on outings, using a motor vehicle.
· Cared for patients by changing bed linens, cleaning, or assisting with their personal care.
· Prepared and served meals to patients according to their prescribed diets, controlled their glucose and Blood Pressure.
· Provided patients with help moving in and out of beds, baths, wheelchairs, or automobiles and with dressing and grooming.
· Provided prenatal, intrapartum, postpartum, or newborn care to patients
· Utilized Metric Tapes, Tension Meters, and scales to take mother’s and children’s measurements
· Documented patients' health histories, symptoms, physical conditions, or other diagnostic information
· Consulted and coordinated with healthcare team members to assess, plan, implement, or evaluate patient care plans
· Taught expecting and new mothers to prevent and treat potential issues around malnutrition
· Ability to communicate effectively with patients, doctors, coworkers, and staff in French, Creole, and English
