Nearing 9 years in the health care field, my nursing career began with the decision to apply for nursing school in 2014 after a year and a half of Early Childhood Special Education at the local community college. I worked night-shift on the Neurology/Stroke unit for over 3 years while attending Mount Carmel College of Nursing, completing the advanced summer program in 2016. Three populations that stuck out to me and became a passion were behavioral, maternal, and community health.
After graduating with my Bachelor's of Science in Nursing and obtaining my RN in 2018, I came across the opportunity to serve the community through Nurse Family Partnership for almost 3 years. It was a dream to have a job that supported what I was so passionate about, but life/family circumstances guided me to explore another job opportunity.
This job opportunity is my most recent place of employment, Quantum Health. I have loved serving the community in a different way and am grateful for the knowledge that I have gained. I aspire to bring this experience to my future place of employment and am eager to continue learning and growing in my nursing career.
Some organizations call this a Case Manager, Nurse Navigator or Care Coordinator. At Quantum Health, they call it a Personal Care Guide (PCG). As a PCG, I served as a primary clinical resource to members navigating a variety of healthcare events and diagnoses by using established screening criteria and recommending care adjustments. I partnered with Pod leaders and clinical staff to ensure consistency of clinical activities. I triaged member questions and acted as a role model and coach. My caseload consisted of members in the high risk pregnancy and infancy population. I additionally served some of those in the behavioral health, cancer, and general population. I provided support and assistance to members during their life-altering experiences by navigating them through things like insurance benefits, billing/claim/medication/equipment issues, health care decisions, provider collaboration, etc. I provided them with any identified resource needs as well.
Having such a passion for community and women's health, I started my nursing career as a Nurse Home Visitor with Nurse Family Partnership at Nationwide Children's Hospital. I provided comprehensive community health nursing services to women and their families in their homes up through the age of 2. I developed close nurse-family relationships that helped to empower women of all walks of life in first-time motherhood by collaborating with staff, providers, and community resources.
My first job in the healthcare field was a Patient Support Assistant at Riverside Methodist on the Neurology/Stroke unit. I assisted Registered Nurses in providing direct patient care to up to 15 patients at a time on my designated nursing unit, sometimes floating to other units. While I loved the personal care I was able to provide for my patients during such vulnerable times, I aspired to make a difference with primary prevention by diving into community health.
