RESPONSIBILITIES

  • 1. Utilizes assessment skills and techniques to determine patient’s needs on admission through clinical observation and physical assessment.
  • Interviews and interacts with the patients and significant others.
  • Performs simple physical examinations such as an inspection, palpation, percussion, and auscultation
  • 2. Completes and documents the nursing admission assessment thoroughly and accurately.
  • 3. Orients the patient and significant others' data on the hospital policies, facilities, Equipment, supplies, and other available services, like pastoral health care services upon admission
  • 4. Provides hygiene and physical comfort measures such as: skin, oral nasal care, wound care, tracheostomy care, proper positioning of patients
  • 5. Assures proper intake of the patient (nutrition/fluid and electrolytes) Maintains on NPO strictly as ordered
  • Gives NG tube and gastronomy tube feeding
  • Administer correct IV fluids with prescribed labeling and regulate accurately to their desired infusion rates
  • Measures and records intake and output in absolute figures
  • 6. Institutes measures for maintenance of proper urinary and bowel functions of the patients Check urination and bowel movement, give laxatives and suppositories as ordered applies hot and cold compresses in case of urinary retention
  • Assist doctors in inserting straight and indwelling catheters
  • 7. Ensures a safe and clean environment within the unit and provides side rails for pediatrics and geriatrics patients, unconscious, restless, and bedridden patients Restraint patients as ordered
  • Adjust the height of the bed properly
  • Properly dispose of contaminated articles
  • Displace proper signs for patients safety (like “Reverse isolation “Limit visitors signs) sees to it that machines such as electrical equipment are properly connected and maintained.
  • Instruct significant others of visiting hours and numbers of visitors allowed
  • 8. Provides therapeutic oxygenation and ventilation demonstrates deep breathing, and turning.
  • And coughing exercises regularly to post-operative, post-intubated, and immobile patients unless contraindicated
  • Maintains the patency of the tracheostomy tube, endotracheal tube, and nasal catheter by proper positioning and suctioning checks oxygenation flow and amount of medical gases
  • 9. Administers medications to patients (oral/parenteral/IV/rectal/vaginal). In case of any untoward reactions, religiously monitor the patients' conditions and implement appropriate nursing interventions.
  • Refers to untoward reactions to the physician and records in the nurse’s notes
  • 10. Assists physicians in the treatment/administration of highly specialized therapy and performing complicated procedures, secures consent.
  • 11. Prepare all necessary equipment and supplies to be used.
  • Monitors patient's condition during and after the procedure performed at beside and refers for unusuality.
  • Documents accurate descriptive records of medical treatment and medications ordered by the physicians.
  • Assists in meeting the patient’s spiritual needs by informing them of available spiritual services and how to obtain them.
  • 12. Calls for emergency codes when necessary
  • Initiates cardiopulmonary resuscitation (CPR)
  • Prepares equipment needed for intubation
  • Replace medicines and supplies in the emergency cart and monitor patients.
  • 13. Receives and gives proper, complete, and accurate change of shift endorsement
  • 14. Checks emergency car content (night shift)
  • 15. Facilities and executes physician’s orders properly and accurately
  • 16. Participates in hospital, departmental, and unit activities like updates and care conference
  • 17. Responds appropriately to patient’s and significant other’s requests and queries
  • 18. Checks the equipment and supplies in the unit and ready them for use at all times
  • 19. Accompanies physicians in making rounds
  • 20. Secures starting medications and intravenous (IV) fluids from the pharmacy
  • 21. Maintains harmonious relationships with other healthcare givers
  • 22. Supervising nursing aides
  • 23. Evaluate the performance of post-graduate trainees in medical-surgical nursing or orienteers
  • 24. Assists the head nurse in orienting new staff nurse
