As a Case Manager, I have been responsible for 30-60 patients. Medication management, IV management, specialty infusions, catheter management, wound care (including wound vacs), patient and caregiver education, and coordinating an interdisciplinary team approach to patient care are some of the things a case manager is responsible for.

As a Clinical Manager, I was responsible for approximately 30 staff and >200 patients. I have experience with QAPI, policy witting/revision, staff education, participating in state surveys and writing correction plans, performance reviews, implementing education/disciplinary plans for staff, patient satisfaction, supply orders, patient care contracts, and many other duties.

Following administration changes that led to organizational restructuring, I have returned to a direct patient care position where I primarily focus on patient admissions and am serving in a temporary case management position as well.

• PRN RN evenings and weekends.
• Worked in addition to full time position with VNA
• Experience with medication administration, IV management, documentation, and other duties.
• Memory care experience.

• Began position as a Float LPN working for UHMG offices across Riley, Terre Haute, Rockville then transitioned to a full time position at Cork Medical.
• Taking patient history, medication reconciliation, vaccine administration.
