• Evaluate the patient’s lab work and diagnostic testing results and report any significant lab levels or abnormal diagnostic results to the doctors.
• In charge of supervising the LPNs and CNAs and allocating the nursing assignments.

• Review medical treatment records of IBM Employees and check whether their Short Term Disability requests may be approved based on MD Guidelines.
• Monitor, assist and provide necessary health teaching and guidance to suspected and confirmed covid 19 cases of IBM Employees in the United States of America and Canada
• Assist IBM Employees with their medical queries.
• Facilitate IBM Employees work modification accommodation requests.
• Facilitate ergonomic equipment requests of IBM Employees.

• Perform prior authorization review of services requiring notification.
• Evaluate medical guidelines and benefit coverage to determine appropriateness of services.
• Submit cases for criteria failures and help facilitate resolutions and approvals.
• Obtain authorizations from multiple insurance carriers for various levels of care.
• Ensure that health care services are administered with quality, cost efficiency, and within compliance.

I handled a team of USRNs (Health Coach Consultants) who call patients with chronic conditions such as end stage renal disease, diabetes, coronary artery disease, hyperlipidemia, cancer, etc. and assist them to have effective clinical discussion with Medicare and non-Medicare members.

• I review and evaluate the phone transactions and the chart documentation of the Health Coach Consultants.
• Provide recommendations on how the Health Coach Consultants can have effective clinical discussion with Medicare and non-Medicare members.
• Communicate with team members about quality issues.
• Collaborate with staff to establish and meet quality standards.
• Participate in team meetings to discuss progress and results, providing ideas and suggestions for improvement.
• Perform root cause analysis to identify and resolve defects, improving quality standards.

• Call patients with chronic conditions such as end stage renal disease, diabetes, coronary artery disease, hyperlipidemia, cancer and conduct clinical discussion about their recent hospitalization, diet, exercise, laboratory results, and help them achieve certain health goals like lowering of A1c, cholesterol levels.
• Provide health teaching as necessary.
• Provide comprehensive, empowering and professional coaching programs so members remain motivated to attain personal health and fitness goals.
• Assist clients in finding new ways to inspire healthier habits.
• Record health data by asking clients to complete assessments.
• Recommend nutrition and fitness modifications to improve overall health of clients.

• Perform nursing procedures (vital signs taking, intravenous line insertions, foley catheter insertions and removal, electrocardiogram, and hemagluco testing).
• Use the nursing process to provide all levels of patient care.
• Observe for signs and symptoms, implement immediate nursing actions, and practice accurate and proper documentation.
• Monitoring for patient’s intake and output.
• Administer appropriate medications through oral, inhalation, subcutaneous, intradermal, intramuscular, intravenous, and rectal routes.
