I am a hardworking, detail oriented, driven individual that has many years of experience in the chemical dependency counseling field. As an individual recovering from substance abuse, I get to use my story to show others that there is a way out from active addiction. I have been clean and serene for many, many years now. With my educational degrees, certificates of completion, licenses in multiple states, knowledge I have gained, and personal experience, I have developed a successful model that I use to assist and support the PEAK ACT clientele, using different type of individual-level interventions based on client assessment. After everything I have been through in life, I have become more and more sympathetic towards others suffering from chemical dependency and behavioral health disorder. I collaborate well with other team members in pursuit of client success and other agencies who seek to offer the same level of care we have at PEAK ACT Program. I implement strategies, utilize the tools, and advocate for clients, all this from my many years of experience. Driven by a strong background in case management and crisis intervention, I excel in fostering client success and safety within Peak Behavioral Health's ACT Program. My adeptness in problem-solving and emotional awareness has significantly enhanced team dynamics and care delivery, underscored by advanced certifications and a dual licensure in substance abuse counseling. For myself, I do not see this as a job but as a calling in my life to help others.

Case Manager

  • As a case manager, I have a number of responsibilities that are vital to our clients well-being and success within our program.
  • Assessments and Evaluations: conduct comprehensive assessments of clients to determine their needs, strengths, and areas requiring support, develop individualized care plans based on these assessments.
  • Coordination of Care: act as a liaison between clients and various service providers, including therapists, doctors, and community resources, ensure that clients are receiving appropriate services and interventions according to their care plans.
  • Support and Advocacy: provide emotional support and practical guidance to clients and their families, advocate for clients to access necessary resources and services, including financial assistance, housing, and employment support.
    Monitoring and Follow-Up: regularly reviewing and updating care plans to reflect changes in clients' needs and progress, track and document client progress, including successes and setbacks.
  • Crisis Intervention: respond to and manage crisis situations, providing immediate support and intervention as needed, develop and implement crisis plans and strategies to ensure client safety and stability.
  • Education and Skill-Building: educate clients about their conditions, treatment options, and coping strategies, facilitate skill-building activities and workshops to help clients develop life skills and achieve personal goals.
  • Documentation and Reporting: maintain accurate and confidential records of client interactions, progress, and outcomes, prepare reports and communicate with other professionals to ensure continuity of care.
  • Collaboration: work closely with a multidisciplinary team, including therapists, social workers, and healthcare providers, to deliver comprehensive care, participate in team meetings and case conferences to discuss client progress and plan future interventions.   • As for the achievements portion, I have acquired certifications and advanced trainings via Healthstream and PEAK Outpatient Program trainings that have assisted me in my professional career by implementing tools and skills obtained. I believe I have improved the team dynamics, fostering better collaboration among the ACT multidisciplinary teams, enhancing the overall effectiveness of care delivery. The PEAK ACT Program has been recognized as one of the top outpatient programs in Las Cruces and the surrounding areas. I recently obtained a (LSAA) License Substance Abuse Associate license from New Mexico to go along with the (LCDC-I) License Chemical Dependency Counselor Intern license I have from Texas. This has all been part of my achievements to date.
