Coordinated Services Team (CST ) and Comprehensive Community Services facilitator (CST)

Job duties are the following:

• Complete comprehensive assessments, including crisis assessments
• Develop recovery plans for improving mental health and/or substance use disorder symptoms
• Participate in team developed Recovery Planning Services
• Facilitate and provide psycho-social rehabilitative services
• Maintain knowledge and ensure compliance with applicable statutes, administrative codes, and agency policies and procedures
• provide wellness and recovery to participants
• Connecting participants to community based service providers
• Building relationships with services providers and participants
• Attend med management meetings with doctors and partipants
• and any other services as deemed necessary

Duties include the following:

• Recruit foster homes for at risk youth
• train and support foster families
• Effectively screen referrals to make appropriate match with foster families
• solicit donations, resulting in an increase of funds and material objects to give youth and/or families
• recruit, train and supervise mentors
• promote the foster care program in numerous public setting
• make public presentations to advocate for the fostercare/mentoring program and LSS overall.

Duties include the following:

• Recruit, train and supervise mentors who worked with at risk youth
• recruit, train and supervise volunteers for runaway hotline
• recruit, train and supervise outreach worker
• solicit donations for families
• Effectively completed grant writing, which resulted in raising funds for multiple programs within the LSS system
• collaborate with other multi-disciplinary teams and agencies
• promote LSS in a public venue such as job fair, vendor tables and public speaking
