· All duties and responsibilities of clinicians

· Administrative duties and responsibilities assigned by the supervisor to augment program managerial oversight

· Authorization to train other program staff

· Participation in chart reviews and documentation compliance oversight

• Provide evidence-based individual and group therapy sessions to clients facing various mental health challenges.
• Monitor client progress and adjust treatment plans as necessary to ensure optimal outcomes.
• Collaborate with other healthcare professionals, including psychiatrists, social workers, and case managers, to coordinate comprehensive care.
• Maintain accurate and timely documentation of client assessments, treatment plans, and progress notes in compliance with organizational policies and regulatory standards.
• Provide crisis intervention and support as needed, ensuring client safety and well-being.
• Engage in ongoing professional development and supervision to enhance clinical skills and stay current with best practices.

• Conduct thorough clinical assessments to evaluate clients’ mental health conditions, strengths, and needs.
• Develop individualized treatment plans based on assessment results and collaborate with clients to establish therapeutic goals.
• Utilize evidence-based assessment tools and methodologies to inform diagnosis and treatment recommendations.
• Maintain accurate and detailed documentation of assessments, treatment plans, and client progress in compliance with organizational policies and relevant regulations.

• Mobilize and facilitate Wraparound Team meetings, empowering and encouraging member participation
• Facilitate the development of behaviorally specific strength-based service plans across range of disciplines, providers and community resources.
• Monitor all treatment to insure adherence to service plan and optimal response.
• Coordinate with current and designated placements to insure optimum transition into appropriate level of care.
• Identify and refer to appropriate community-based services to meet client’s and family needs.
