RN Care Manager with focus on discharge planning for high volume observation unit in currant Care Management role, also cover antepartum unit in Womens .This role involves daily interdisciplinary rounding , patient assessments for discharge planning needs, communication with all team members to facilitate best discharge plan possible for patients being followed.
Initially in this role 2008 to approx 2013 did both utilization review and discharge planning for Womens Services units. 2013 to approx 2015 worked on readmission project with previous Director , with additional responsibilities to review one-day stays , review OR list to verify scheduled surgeries in correct admission status and assisted IDIC nurse to review charts to place DRG sheets and queries on physical charts.

During this time, used Interqual extensively when in UR role, became a qualified IQ instructor in 2012-2014.
Extremely proficient in using EPIC and assisted other NTX Care Management departments on their go-live to EPIC by physically being on their campus.
