.Complete pre-admission assessment on patients to gather clinical and financial information and identify optimal service location.

.Establish and maintain each Center's identified census goal.

.Create working relationships with social workers/case managers at assigned hospitals.

.Knowledge of Medicare, Medicaid, Managed Care, Subacute and Long Term Care.

Knowledge of Epic, Careport and Allscripts electronic referral systems.
