Compassionate emergency care for unsheltered patients with pre-existing health conditions. Providing individuals experiencing homelessness with the service guidance, support and advocacy needed for their health in a kind, caring and trauma informed way. Safely assisting and engaging and or performing de-escalation using a "no-force", harm reduction approach to assess risk, assess patients immediate health needs, and provide appropriate interventions.

• Assess patients condition and report assessment findings to the Medical care provider, Registered Nurse or Paramedic.
• Check the facility's Go-Bag and ensure all items are stocked, expiration dates are compliant and the bag is securely locked and all information is recorded accurately.
• Provide first aid treatment and or basic life support per protocols and policies.
• Assist if necessary to transfer residents to designated community care facilities or EMS.
• Wellness checks on all patients and site rounds every hour.
• Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options.
• Maintained patient vital signs, including blood pressure, respiration rate, pulse rate, temperature, oxygen saturation level and pain levels.
